Home Based Primary Care (HBPC)

Mission/Vision 

To honor America’s Veterans by providing exceptional health care in the home setting that supports the Veteran’s changing needs and preferences throughout the course of their life. 

Scope Service

The goal of HBPC is to provide primary care in the home.  The program helps Veterans remain independent and have a better quality of life. HBPC provides care for 150 Veterans.  

HBPC Veteran Services

  • Nurse case management.
  • Home visits from HBPC staff.
  • Telephone contact.
  • Help with medications.
  • Assistance with health issues.
  • Health and self care education.
  • Family and caregiver support.

  • Physical Therapist
  • Psychologist
  • Pharmacist
  • Program Support
