Southern Oregon Rehabilitation Center & Clinics (SORCC)
Primary Care
Mission/Vision
To provide Veterans of all ages the ability to enroll into the VA healthcare system and, if accepted, to provide a full continuum of healthcare services based on “medical need." Medical need means any medical resident or outpatient care that will:
- Promote, preserve, or restore health
- Has been prescribed by a VA clinical care provider
- Is consistent with generally accepted standards of clinical practice
Scope of Service
Primary Care includes Residential Care at VA SORCC and Ambulatory Care Clinics located at the VA SORCC; Klamath Falls CBOC; Grants Pass Rural Health outreach clinic; and the Lakeview Rural Health extension clinic. All are designed to provide primary healthcare services to enrolled veterans based on medical need. Primary Care Providers (PCPs) will diagnose and treat medical problems within the scope of their training and make appropriate and necessary referrals for specialty care and diagnostic studies to other VA and/or non-VA facilities depending on eligibility for such services.
Enrollment into the VA Healthcare System means that Veterans are eligible for a comprehensive healthcare benefits package of resident and outpatient services, which include:
- Preventive services, including immunizations, screening tests, and health education and training classes
- Primary medical care
