Mission/Vision  

Social Work Services staff is committed to eliminating psychosocial complications as significant barriers to health care interventions for Veterans, their families and caregivers. It assists in providing a continuum of care that allows Veterans to function at the highest possible level thus obviating their need for placement in a higher level of care.                           

Service

Social Work Services consists of staff committed to working with Veterans towards ending their homelessness and achieve the highest level possible of self-sufficiency through the Homeless Programs. The Caregiver programs also provide a continuum of care for Veterans and allows them to function at the highest level possible thus obviating their need for placement in a higher level of care facility. This is accomplished with the following programs:

  • HUD VASH is a collaborative effort between Veterans Affairs and the Department of Housing and Urban Development. Its primary goal is to move Veterans and their families out of homelessness. A key component of the program is VA’s case management services designed to improve the Veterans physical and mental health and enhance their ability to live in safe and comfortable permanent housing.
  • Grant Per Diem provides a bed in the community for homeless or soon to be homeless Veterans trying to get back into the workforce. The Grant Per Diem Liaison assists in linking to services that will assist them in becoming self sufficient.
  • Enhanced RANGE specializes in providing outpatient mental health treatment in community settings. It targets the Veteran population of those with severe and persistent mental illness that has not responded to traditional forms of outpatient care and is homeless. The program goals are to reduce the need for hospitalizations, improve symptom and medication management, strengthen community support systems, increase independence in self care and living skills, and obtain housing.
  • Homemaker/Home Health Aid Program assists Veterans and their families with contracted care providers in their home. They help them with care needs and relieve the burden on the family caregiver.
  • Caregiver Support Program provides contracted caregivers and pays family caregivers as they care for Veterans in the home.
  • Volunteer Caregiver Program provides volunteer caregivers to visit Veterans in their homes. They support Veterans and help relieve the burden on the family caregiver.
  • Respite Program provides a respite for Veterans through a day care model or a 24 hour respite program for up to 30 days per calendar year to relieve the burden on the caregiver.
  • The VA SORCC is committed to our troops from Operations Enduring Freedom, Iraqi Freedom and Operation New Dawn and ensuring that they have the health care and benefits they need. We can also provide priority health care and benefits assistance even if you are on active duty or an activated member of the National Guard or Reserve.
